| How do I get my new prescriptions filled? If this is your first time ordering, fill out the new member enrollment form, include a check for your copay and your new prescriptions with the main participant's identification number on the back and mail to Integrated HMO Pharmacy Services - PO Box 369, Boys Town, NE 68010. If you've filled prescriptions with us before, no form is required. Simply write the identification number of the main participant on the back of the prescription and mail it to Integrated HMO Pharmacy Services - PO Box 369 Boys Town, NE 68010. | I have an existing prescription; can I transfer it? As long as there are refills remaining, we will make every effort to transfer your existing prescription, however, you could experience delays. If it is possible, sending a new prescription is the quickest way to receive your medication, since we can begin processing immediately. To transfer an existing prescription to Integrated HMO Pharmacy, please contact the pharmacy staff at 1-800-633-7928 and a member of the pharmacy staff will provide you with further instructions. | Can my doctor call in my new prescription? Absolutely. Your doctor may call our toll-free number to call in a prescription. The only exception would be some controlled drugs. Some of these medications require a new original prescription. Top |

| How many refills can my doctor write for? If your prescription is written for at least a 90-day supply, your doctor may write for 3 refills, which will supply you for one year. Controlled drug prescriptions, however, can only be refilled once since they will expire 6 months from the day they are written. Top |

| How long are my prescriptions good for? Most prescriptions expire one year from the day they are written. Controlled drug prescriptions expire 6 months from the day they are written. Top |

| When will I receive my medications? Your order will be processed within 3-5 business days of receipt. Please allow 14-21 days from the date you mail your order until you get your medication. This allows for normal postal delivery time. When ordering refills, you may expedite your order by requesting refills by phone at 1-800-633-7928 or via the Internet at http://ihmo.pti-nps.com or via e-mail to IHMO@pti-nps.com. When ordering by phone or via Internet, you will need Visa or MasterCard for your copay. |

| Do I need a new prescription every time? If your prescription has refills remaining, you do not need a new prescription every time. It's a good idea to request that your doctor write your prescription for the maximum days supply plus three (3) refills. This should supply you for a year. (Exceptions would be controlled drug prescriptions, which cannot be refilled or expire six months from the day they are written.) Top |

| Do I have to fill out a form with every new prescription? No form is required with new prescriptions after the initial order. | How do I order additional forms? You only need to fill out the enrollment form once. You do not need additional forms even for new prescriptions. The form you fill out with your initial order provides us with the health and insurance information we need on your family. Top |

| Can I phone in my refills? Absolutely. Have your rx numbers (found on your bottle or prescription reorder form) ready and then call 1-800-633-7928; or you may leave a message with the prescription refill center and your order will be processed. Top |

| Do you send my refills automatically? No. We don't assume that you will be continuing on this medication. You must mail in your prescription reorder form or a brief letter stating the rx numbers or the name of the medications that you need refilled. | If my prescription is out of refills, can you call my doctor? Yes. We will make every effort to obtain approval from your doctor to refill your medication. Please be aware that you may experience delays if we have difficulty contacting your doctor. Top |

| Why didn't I receive my order all at the same time? Most plans require that the member use between 65% and 75% of their last refill before they will allow us to refill that prescription. This could be why you received your order in two shipments. If it is too early to fill one of your prescriptions, you will be notified, and we will fill it on the earliest date allowed by your insurance provider. Top |

| Who decides my copay? Copay, as well as maximum days supply is decided by your plan sponsor or employer. Top |

| What drugs are covered? Integrated HMO Pharmacy does not determine what drugs are covered by your plan. If a situation occurs in which a drug is not covered, you will need to contact the National Pharmaceutical Services Customer Service Center at 1-800-546-5677 and will answer any drug coverage or copayment questions that you may have.
